Calistoga Family Center seeks a highly creative professional to join our team of dedicated family advocates. Together, we work to enhance families’ capacity to support the growth and development of all family members, including adults, youth and children.

Calistoga Family Center cultivates healthy families by providing integrated health and family services for a stronger Calistoga. In 1999, Calistoga Family Center (CFC) was formed by concerned community leaders who understood that healthy families are the foundation of a strong community. CFC serves as the heart of community services designed to improve family life, especially for overburdened families. CFC is the only locally based social service agency in Calistoga. Because nineteen different agencies are housed in the Center over the course of a week, services to families are delivered seamlessly and are fully integrated.
Overall Responsibilities
The Family Services Coordinator (FSC) is the lead staff member for the Strong Families program. The FSC provides extensive case management and referral information to Calistoga families. He/she works collaboratively with Family Center staff and service providers, school personnel, and the staff of Napa County Health and Human Services. The FSC is responsible for approximately 35 cases.
Summary of Duties and Functions:
• Use the principles of Integrated Team Case Management to support Calistoga families to meet their goals
• Develop, direct, and provide on-going case management and follow-up for families that is preventive, supportive, culturally appropriate, empowering, and builds upon student and family strengths
• Develop and lead multi-disciplinary team meetings to coordinate services, monitor progress, and ensure timely follow-up
Essential Qualifications:
• Be highly creative, possess a positive attitude and demonstrate a strong sense of commitment to families
• Good written and verbal skills in English and Spanish (approximately 80% of clients are Spanish-speaking)
• Bachelors degree in social work or nursing or related field with relevant experience
• At least two years experience in a family agency, outpatient clinic, child guidance clinic, or behavioral health program demonstrating previous work with children and families
• Ability to work in and lead a team to provide service to individuals, families, and groups
Desired Qualifications
• Masters degree in social work, nursing or related field—LCSW or MFT licensure is a plus
• Knowledge of local community resources and service providers
• Experience in school systems and the school environment
Employment Details
• 40 hours per week, Monday – Friday
